Spanish‑Speaking Team Nurse – Outpatient Primary Care

About the role

The Team Nurse provides direct, high‑quality patient care in an outpatient primary‑care setting for Beth Israel Deaconess HealthCare patients. Reporting to the Practice Manager and Senior Director of Nursing, the role supports a Spanish‑speaking patient population and works closely with providers, medical assistants, and community resources.

Key responsibilities

• Coordinate and deliver clinical management for a broad outpatient population, including preventive care and chronic disease management.
• Assist providers with examinations, treatments, procedures, and documentation in the Electronic Health Record (EHR).
• Conduct annual wellness visits, chronic care management, and telephonic triage.
• Manage INR monitoring and Coumadin education using a standardized protocol.
• Administer vaccines and medications per provider orders and BIDHC policies.
• Perform wound care, point‑of‑care testing, and other nursing interventions as needed.
• Schedule tests, referrals, and ensure follow‑up of abnormal results.
• Educate patients and medical assistants on health topics and preventive strategies.
• Oversee inventory and ordering of clinical supplies and equipment.

Required profile

• Current RN or LPN license with experience in outpatient primary‑care or family‑medicine settings.
• Fluency in Spanish to communicate effectively with patients.
• Ability to collaborate with providers, medical assistants, and community partners.
• Strong organizational skills and attention to detail.
