Per Diem Hospice RN – Outpatient Care

About the role

Join a compassionate hospice team providing outpatient care on a flexible per‑diem schedule. You will deliver nursing expertise to patients whose conditions cannot be managed at home, supporting both patients and families during critical moments.

Key responsibilities

• Coordinate and implement individualized Hospice Plans of Care for assigned patients.
• Perform ongoing assessments, develop nursing interventions, and evaluate patient responses.
• Educate patients and families about disease processes, pain and symptom management, and provide emotional support.
• Collaborate with an interdisciplinary hospice team under physician direction, adhering to Arizona nursing regulations.
• Assist with patient movement (up to 300 lb) and perform required physical tasks throughout the shift.

Required profile

• Graduate of an accredited nursing program (associate degree or diploma).
• Active Arizona RN license or compact state RN license.
• Level One Fingerprint Clearance Card (obtainable within 20 days of hire).
• Arizona driver’s license and Food Handler’s Card.
• Current BLS certification (American Heart Association or Red Cross).
• Minimum age of 21 years.
• At least one year of acute‑care or hospice nursing experience.
