Hospice RN Case Manager – 7-on/7-off Schedule

About the role

We are seeking a full-time Hospice RN Case Manager to provide nursing care and coordination for patients in the Corpus Christi and Kingsville area. The position follows a 7-on/7-off schedule, with evening and overnight on-call coverage and routine field visits. You will travel to patient homes, skilled nursing facilities, and assisted‑living communities, ensuring timely and compassionate care.

Key responsibilities

• Conduct nursing assessments and routine hospice visits in patients’ homes, nursing facilities, and assisted‑living communities.
• Monitor symptoms, medications, and changes in condition, updating individualized plans of care.
• Educate patients and families on symptom management, disease progression, and end‑of‑life expectations.
• Coordinate care with physicians and the interdisciplinary hospice team and respond to urgent after‑hours needs.
• Maintain accurate and timely documentation in the electronic medical record (EMR) system.
• Document patient interactions and care plans in compliance with hospice regulations.

Required profile

• Active, unrestricted RN license and reliable transportation with a valid driver’s license.
• Comfortable with the 7-on/7-off schedule, overnight call, and independent field work across Corpus Christi and Kingsville.
• Strong clinical assessment, nursing judgment, and communication skills with patients, families, and care teams.
• Willingness to support patients and families through serious illness and end‑of‑life care.
• Hospice experience preferred but not required; strong nursing foundation is essential.

What we offer

• Base salary of $80,000–$83,000 per year.
