About the role

We are looking for a qualified medical professional with strong English communication skills to conduct telephone screenings and medical assessments for patients across New York State. The role focuses on gathering accurate health information and preparing clinical documentation that supports eligibility decisions for home care, durable medical equipment, and other services.

Key responsibilities

• Conduct telephone screenings with patients, collecting detailed medical histories, current conditions, medications, and functional limitations.
• Evaluate the information against established guidelines and service requirements.
• Create and maintain organized patient charts and supporting documentation.
• Identify missing or inconsistent data and follow up with patients as needed.
• Communicate professionally and compassionately with patients, caregivers, and internal team members.
• Meet quality, accuracy, productivity, and turnaround‑time standards.

Required profile

• Medical education, training, or professional experience in a healthcare‑related role.
• Advanced verbal and written English proficiency.
• Strong understanding of medical terminology, patient history collection, and clinical documentation.
• Excellent telephone communication and patient‑interviewing skills.
• Ability to manage multiple cases and meet deadlines.
