Functional Wellness and Rehabilitation Educator

About the role

Habitat Health is seeking a Functional Wellness and Rehabilitation Educator to lead the onboarding and training of clinical and support staff at its PACE centers. The role is remote‑based with extensive regional travel to support new center launches across California.

Key responsibilities

• Design and deliver on‑site didactic training for Physical Therapists, Occupational Therapists, Activities Coordinators, and Personal Care Attendants.
• Oversee competency reviews, sign‑offs, skills validation, and preceptor checklists throughout the onboarding process.
• Act as the field‑based owner of training fidelity, ensuring consistent workflows, documentation, and best practices.
• Partner with Practice Operations, Center Leadership, People Team, and other functional educators to optimize training materials and operational resources.
• Drive process improvement, program development, and regulatory compliance to support high‑quality participant care.

Required profile

• Experience in functional wellness, rehabilitation, or clinical education within a healthcare setting.
• Proven ability to train and mentor PTs, OTs, and allied health staff.
• Strong organizational skills to manage onboarding, practicum, and shadowing activities.
• Residency in California and willingness to travel up to 75% of the time.
