Director of Partnerships

About the role

Habitat Health is seeking a Director of Partnerships to lead and expand its collaborative network as the organization scales its PACE model for older adults. This senior, cross‑functional role will shape partner strategy, oversee operational workflows, and serve as the primary liaison between Habitat and its external stakeholders.

Key responsibilities

• Develop and manage partner relationships at all levels, from day‑to‑day operations to senior executive engagement.
• Strengthen partner‑facing workflows and tools to improve care delivery, patient experience, and market performance.
• Identify and solve operational challenges, derive systems‑level insights, and design systematic solutions.
• Build a scalable partnership management structure to support new center launches and adapt across partner archetypes and markets.
• Adapt Habitat’s operations to support expansion into new markets and meet local partner needs.
• Deliver executive updates to Habitat and partner leadership, aligning internal initiatives with external commitments.
• Support broader strategic initiatives across the enterprise, especially those impacting partnerships.

Required profile

• 6–10+ years of experience in the healthcare industry focusing on strategy, operations, partnerships, business development, or consulting.
• Proven ability to drive operational execution and set vision with senior stakeholders.
• Strong communication skills and sound judgment in ambiguous operational problems.
• Track record of success across cross‑cutting priorities and complex organizational dynamics.
