Case Manager – Housing Support

About the role

Share’s Affordable Housing & Stability programs use the Housing First model to move the most vulnerable houseless individuals into permanent housing and provide case management support to help them stay housed. The Case Manager works directly with clients to secure housing, overcome barriers, and promote long‑term stability.

Key responsibilities

• Maintain an active caseload, conduct eligibility assessments, and process monthly reporting.
• Assist clients with housing navigation, lease compliance, rent payment, and relationship building with landlords.
• Connect clients to behavioral health services and develop personalized crisis prevention plans.
• Compute benefits and payments accurately, ensuring compliance with program guidelines.
• Participate in committees, policy improvement initiatives, and ensure departmental compliance with funder requirements.

Required profile

• Bachelor’s degree (or equivalent 4‑6 years of relevant experience) with 2‑3 years of case management experience.
• Demonstrated professionalism, strict confidentiality, and a collaborative attitude.
• Strong organizational skills, attention to detail, and ability to meet deadlines in a fast‑paced environment.
• Excellent verbal and written communication skills.

Required skills

• Experience with or willingness to learn Peace Health EPIC case management system.
• SOAR Advocate Training certification (or willingness to obtain).
