Adult Nurse Care Manager (Remote)

About the role

We are seeking an experienced Nurse Care Manager to join our Care and Case Management team. The role is fully remote and focuses on providing holistic, virtual support to adult members throughout their healthcare journey.

Key responsibilities

• Deliver coordinated, patient‑centered virtual care management via telephone or video to improve health outcomes.
• Create and maintain impactful care plans in partnership with members and a multidisciplinary care team.
• Guide members through complex medical conditions, treatment pathways, benefits, and the broader healthcare system.
• Collaborate with local providers to ensure seamless, coordinated care.
• Provide compassionate, longitudinal follow‑up and build supportive relationships with members and families.
• Assist during acute episodes such as hospitalizations or rehabilitation stays, offering case‑management support.
• Coordinate clinical and social resources to address members’ holistic needs.

Required profile

• Bachelor of Science in Nursing (BSN).
• Minimum 5 years of nursing experience.
• At least 2 years of experience in care management.
• Residency in a compact NLC state with an active compact RN license in good standing.
• Willingness to obtain and maintain licensure in multiple states.
• Availability to work 9 AM–6 PM PST.
• Spanish speaking is desirable.
• Experience working remotely and proficiency with multiple computer/medical record systems.
• Strong empathy and adherence to HIPAA security regulations.

Required skills

• Proficiency with electronic health record (EHR) systems.
