Clinical Pharmacist – Ambulatory

About the role
The Ambulatory Clinical Pharmacist will educate patients, families, and the care team on safe medication use while delivering cost‑effective therapy across the outpatient setting. This full‑time position works closely with interdisciplinary teams to ensure seamless transitions of care from admission through discharge and follow‑up.
Key responsibilities
- Educate and counsel patients, families, and staff on medication safety and effectiveness.
- Provide timely, cost‑effective medication therapy and participate in stewardship and restricted‑use programs.
- Engage in direct patient care, interdisciplinary rounds, and transition‑of‑care activities.
- Supervise, mentor, and evaluate pharmacy technicians, interns, students, and residents.
- Identify, report, and resolve medication safety events, adverse drug reactions, and errors.
- Utilize EHRs, pharmacy informatics, and medication automation to optimize clinical decision support and documentation.
- Perform compounding, dispensing, verification, and documentation across central, satellite, and IV areas.
- Review medication orders, conduct pharmacist consults, and perform pharmacokinetic dosing and monitoring.
- Participate in quality‑improvement, regulatory compliance, and accreditation activities.
Required profile
- PharmD (Doctorate) preferred; Bachelor’s degree required.
- At least 1 + year of hospital pharmacy experience preferred.
- Additional licensure or certification may be required.
- Strong organizational, verbal, and written communication abilities.
