Pharmacist – Long‑Term Care (Houston, TX)

About the role

PharMerica is seeking a full‑time Pharmacist to join its closed‑door pharmacy team in Houston, TX. You will work in a non‑retail environment supporting long‑term care and senior living residents, ensuring safe and effective medication therapy.

Key responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions, and route.
• Provide clinical recommendations and optimize medication therapy.
• Maintain up‑to‑date resident profiles, including medical history, labs, and all medications.
• Order, review, and communicate required laboratory tests for high‑risk medications.
• Collaborate with physicians, nurses, and other healthcare professionals.
• Document all communications and interventions in the medical record.
• Enforce policies for safe handling, dispensing, and oversight of medications.
• Ensure compliance with state and federal regulations (CMS, DEA, FDA).
• Report medication errors and adverse events to the Pharmacy Manager.
• Educate staff on safe medication practices and perform additional pharmacy tasks as assigned.

Required profile

• Bachelor’s degree in Pharmacy (Pharm D preferred).
• Current, valid pharmacist license in the state of practice.
• One year of experience in long‑term care is desired.
• Strong clinical judgment and effective written and verbal communication.

Required skills

• Knowledge of drug‑therapy guidelines and medication safety practices.
• Familiarity with regulatory standards (CMS, DEA, FDA).
• Proficiency with electronic medical records and pharmacy information systems.
