Bilingual Clinical Pharmacist (PharmD)

About the role

Scene Health is seeking a bilingual Clinical Pharmacist (PharmD) to join its Patient Solutions team. You will work remotely with a multidisciplinary care team to improve medication adherence through video‑based coaching, medication therapy management, and personalized patient outreach.

Key responsibilities

• Conduct patient outreach, handle inbound calls, and deliver pharmacist services as part of the Scene program.
• Perform Medication Therapy Management (MTM) services, including Comprehensive Medication Reviews (CMRs), targeted reviews, medication reconciliation, and prior authorization reviews.
• Provide asynchronous video consultations and check‑ins to reinforce clinical information and guide patients into clinical programs.
• Assess medication regimens for unnecessary drugs, dosing issues, adverse reactions, effectiveness, and adherence challenges.
• Document clinical recommendations, care notes, and follow‑up actions in a clear, timely manner.
• Communicate recommendations to provider offices and ensure they are enacted.
• Collaborate with nurses, health coaches, product managers, and client‑facing staff to coordinate care.
• Meet productivity and performance targets and help develop operational policies for pharmacy services.

Required profile

• PharmD degree with active pharmacist licensure.
• Bilingual proficiency (English and a second language).
• Strong patient‑centered communication and ability to motivate and engage both patients and providers.
• Experience delivering MTM services and conducting comprehensive medication reviews.
• Demonstrated thought leadership in optimizing pharmacy workflows.
